The Middle Fabius River is an approximately 75–76 mile (ca. 120–122 km) long river in northeastern Missouri, United States. It is formed in Scotland County by the confluence of the North Fork Middle Fabius River and the South Fork Middle Fabius River (both originating in Schuyler County), then flows generally southeastward through Knox and Lewis counties before joining the North Fabius River in southeastern Lewis County. Largely unchannelized and naturally meandering with excellent forested buffers and diverse features such as pools, riffles, snags, and drift piles, it offers one of the best fish habitats in the region, particularly known as an outstanding stream for sizable channel catfish.
